Getting started with Rise is easy. Once you complete the new client assessment form, we will contact you to book a 45 minute introductory clinical session. We'll talk about any injuries or preexisting conditions, your goals, how to submit to insurance (if applicable), and build a treatment plan together.

Specialized Programming

Specialized programming by Dr. Katie: for individuals managing injuries or illnesses which are inhibiting life experiences or make other pilates offerings unsafe.


This offer includes carefully dosed, pilates-based exercise, manual bodywork, and ongoing assessments. Clients can expect 60-90 minutes of quality time with Dr. Katie.

Out of Pocket Costs

Rise Integrative (Katie Daniels Buyniski DPT, P.C.) is a participating provider with Blue Cross Blue Shield (BCBS).  If you would like to know about your particular coverage, we are happy to assist can be reached at admin@riseintegrative.com or 219-595-RISE


If Rise Integrative is considered out of network with your plan, you can still submit receipts for physical therapy care to your insurance or HSA/FSA for reimbursement. All therapy sessions are 1-on-1 with a licensed Doctor of Physical Therapy.


Referrals/Questions

Indiana is a “direct access” state for Physical Therapy. You can be evaluated and treated by a physical therapist for 42 days without a referral from your individual provider (physician, podiatrist, psychologist, chiropractor, dentist, nurse practitioner, or physician assistant).


Your insurance plan may require you to have a referral or a prescription for physical therapy for the service to be covered.
